LDCSB Next Year Student Transfer Form 2025-2026

Welcome to the London District Catholic School Board’s Current Student Transfer Form.

The Current Student Transfer Form is required for active students transferring between LDCSB schools. This form cannot be saved for later submission, so please make sure it is fully completed and submitted.

Please follow the steps below to register with the London District Catholic School Board:

1. For parent(s)/guardian(s) transferring a student already registered for JK to 12 this transfer form needs to be completed. If the student is not yet registered please fill in the LDCSB Current Student Transfer Form.
2. Please Note: All required information is denoted as 'Required.'
3. You will have an opportunity to review all your information on the 'Summary' page prior to final submission.
4. You cannot save this form and return to complete it at a later date. If you do not complete the form, the form will be NOT be saved. Ensure you have all the required documents ready to complete the form.
   
5. You will have an opportunity to print a copy of your transfer form on the 'Submission Confirmation' page.
6. Once your Current Student Transfer Form has been electronically submitted, you will receive an email confirmation from noreplyregistration@powerschool.com

Requesting a Transfer More Than One Child

A Current Student Transfer Form must be completed for each child in your family that is requesting to transfer between LDCSB schools. Once you have successfully submitted one Current Student Transfer Form, you may complete for an additional student by accessing the link to this form again.

If your student has shared custody between legal guardians who live at separate addresses, both legal guardians will need to agree to the registration.
If a student is already registered at one school and custody arrangements have not been finalized, and the legal guardians are not in agreement, the student is not able to register at another school until legal direction has been received. A custody order will need to be provided to the school.

An official Birth Certificate or Passport is required for Proof of Age and Legal Name.

Please enter the student's legal name as written on a legal document such as the birth certificate or Passport.

Two registered documents dated within the last 6 months are required for Proof of Residency at home address

Regular property address audit verifications are conducted to ensure eligibility to attend designated school.

Is the student's mailing address different from the property address above?

Please ensure to update the primary and secondary contacts for students who are transferring to reflect their current details. If there are additional changes needed for Contact or Emergency Contact information due to a transfer, kindly reach out to the respective school for assistance once transfer is approved.

To be eligible to attend a Catholic Elementary School with the London District Catholic School Board, you must complete the school support designation form via an online portal with MPAC (Municipal Property Assessment Corporation).

Once completed, please ensure you upload an E-mail Confirmation of your School Support Form as part of your Current Student Transfer Form with the London District Catholic School Board.

Students will NOT be registered if the Direction of School Support Form has not been completed.
